Australian Defence Medal - Use this form only if you are applying for the Australian Defence Medal.
Issue of Medals - Use this form if you are applying for any medals other than the Australian Defence Medal.
Posthumous Issue of Medals - Use this form if you are applying for any medals (including the Australian Defence Medal) for a deceased person.
Issue of Replacement Medals - Use this form if you are applying for the replacement of medals which have previously been issued but have been lost, stolen or damaged (including the Australian Defence Medal). Please note - family members or next of kin cannot apply for replacement medals for a deceased person.
Approval to Accept and Wear Foreign Awards - Use this form to apply for approval to accept and wear foreign service awards, or to wear foreign service awards received while serving as a member of a foreign defence force.
UN Medal - Use this form to apply retrospectively for the United Nations Medal. Please return the form to the Directorate of Honours and Awards for verification.
